beCAUSE 4 PAWS understands that the decision to surrender your animal is difficult and appreciates your time in submitting all required information.

The information that you provide will help the rescue place your animal in the best foster and/or forever home. Please respond thoroughly and honestly.

All components must be completed in their entirety for the Owner Surrender Inquiry to be processed.

The Owner Surrender paperwork for dogs and cats includes all of the following:

1. All medical records to date, including dates monthly preventatives were last given.

2. Current photos of your animal (at least 5 within the last 2 weeks)- Photos should clearly show your animal without people of other animals.

3. Owner Surrender Questionnaire

4. DOGS ONLY- A comprehensive training evaluation completed by a professional and licensed trainer and/or behaviorist that includes specific recommendations for working with the dog. Please contact the rescue at ownersurrender@because4paws.org for recommendations.
